Request for B.E.S.T. Academy ID Card
School ID cards are digitally made so it is very important a digital picture of the student be submitted using the link to the form below. The first student ID card will be run for students who request before September 2nd. After the initial run, ID's will be processed on the 1st day of each month.
Please fill out the ID Request form below to receive your B.E.S.T. Academy ID card.
You must include the following items to create a digital Student ID card:

Student’s full name

Date of birth

1”x1” passport-size digital photo. The photo must meet the following guidelines:

Headshot only – facing the camera on white background

No sunglasses
-
No hats or headphones
-
No costumes
-
No making faces - just smile!

Once you complete the necessary information and submit, your student ID will be mailed to your address on file.
